THE FOOD SHELF News:
With recent changes in Minnesota legislation, our food shelf has had to change our name from the Therapeutic Food Pharmacy to “THE FOOD SHELF” @ HCMC.
THE FOOD SHELF was the recipient of a James B. Linsmayer Foundation ($20,000) Grant. The funds will be used to support the activities of THE FOOD SHELF.
A huge boost to the program came January 8th when KARE 11 featured THE FOOD SHELF as its January Volunteer Opportunity of the Month on their KARE 11 Backyard News segment.
THE FOOD SHELF also kicked off our new website at http://www.thefoodshelf.com so check it out! We also now have a Facebook page so please look for us there.
Food Assistance EPIC Referral Order:
For patients identified in need of food assistance, a referral order can now be generated in EPIC. The EPIC order, with the contact’s name and phone number, will be auto-faxed to Second Harvest Heartland. Second Harvest Heartland’s staff member will call to assist them in signing up for SNAP (food stamps, utilized by only about 65% of those eligible for the program in Minnesota) and provide individualized screening for eligibility for other food assistance programs (WIC, NAP). They will also provide information about relevant neighborhood-specific food shelves and kitchens and on-going case management that will follow progress of those contacted in terms of successfully connecting to resources. Our partners at Second Harvest Heartland are geared up to act on behalf of our families, and are anxious to test this intervention for possible replication in other healthcare systems. Please help us to make it a success. Orders may be entered by providers or social workers.
You need to only enter an order “Referral for Food”. The form asks you to check a box indicating that the contact person has asked to be contacted and provide the name, phone number, and preferred language of the contact. That’s it – sign it and send it on its electronic journey to a trained SNAP outreach staff member.
